Deviated Nose Surgery (Septoplasty)


Overview of the surgery:
Deviated Nose (Septoplasty) is an operation to correct a deformity of the nasal septum. The usual purpose is to improve nasal breathing, but it may also be performed to allow adequate examination of the inside of the nose for treatment of polyps, inflammation, tumors, or bleeding. When the nasal septum is deformed, there is no medicine that will cause it to be straightened, so surgery is the only solution to this problem.
A small incision is made inside the nose. The mucosal lining of the septum is detached from the cartilage and bones of the septum. The deviated portions of the septum are removed or straightened. The nasal lining mucous membrane is replaced.

Duration of the operation:
Septoplasty surgery to straighten the septum or remove the obstructing anatomy takes about an hour of operation.

Post operative Care:
Splints or packs are placed in the nose for a few days to help the septum to heal into its new position. These splints / packs have a straw that allows the patient to breathe from the nose and makes swallowing much easier. There are no external bruises or swelling of the eyes because the outer nasal bones are not broken or interfered with as in cosmetic nasal surgery. If an antibiotic is prescribed then it should be strictly followed to prevent infection.  Saline irrigation of the nasal cavity helps in speeding up healing process.

Recovery period:
The septoplasty recovery time normally ranges from one week to three weeks. Patient is advised to blow his nose only after a day or two and that too very gently. Only after a week or two are patients asked to blow their nose normally.

Possible risk and complications:
The undesirable results that may occur include: a hole in the septum, failure to completely improve breathing, bleeding (usually easy to control), wound infection, nasal crusting, and very rarely, a change in appearance. . There are risks with all surgical procedures. You should discuss all risks with your cosmetic surgeon.

Anesthesia used:
The procedure is performed with local, IV sedation or general anesthesia.

Hospital admission:
The patient can be discharge from the hospital on the same day of the surgery.

Overview of the surgery:
Alar Resection is a cosmetic surgery procedure of the nose that narrows the wide flaring and large opening of the nasal nostrils.
The alar of the nose is reduced and suture in its position, aim is to make the balance of nasal tip and alar of the nose.

Duration of the operation:
The procedure usually takes around 1 hour.

Post-Operative Care
Splint the nasal tip with 3-4 mm thin strips of waterproof tape. Steri-Strips also can be used. Run the strips along each side of the nose and gently wrap them under and around the tip. As all sutures are absorbable, none require removal. Revision surgery should not be considered before at least 6 months have elapsed since the original operation.

Recovery period:
Normally, you will feel some pain and discomfort at the nose area during the first days after the operation but it can be alleviated by the medications prescribed. After 2 or 3 days, you will feel no pain but swelling in the nose and eyelid area will persist. Swelling and bruising around the eyes will start to disappear within a week and will completely subside for about 2 weeks. You can go back to work after 1 or 2 weeks, but avoid strenuous activities that can increase pressure to the face, which can prolong swelling and bruising for the next 2 to 3 weeks. After the removal of the bandage and stitches, the patient must take care of her/his nose not to be injured until it is completely healed. The frames of glasses should not be allowed to rest on the bridge of the nose for three weeks. A little bleeding is common during the first few days following surgery; patient may continue to feel some stuffiness for several weeks. Patient must try not to blow nose for a week or so, while the tissues heal.

Possible risks and complications:
Serious complications are rare. The most common complication is patient dissatisfaction. Other complications include: hematoma, airway obstruction, injury to the nose, infection, excessive swelling and bleeding.

Anesthesia:
Using IV sedation (medicine administered through an IV) and local anesthesia (numbing medication administered in the surgical area)

The surgery includes realignment of the alar cartilage by resection and suture, fibroareolar and subcutaneous tissue, tip graft and columellar strut. The nasal tip is positioned high and elegant, methods such as cartilage suture, cartilage grafting and blocking the nasal septum.

Duration of the procedure:
The procedure usually takes around 1 hour depending on the surgical treatment and the extent of reshaping required.

Post operative care:

Recovery period:

The incisions are placed on the rim of the alar of nose. Soft tissues of the nose are then separated from the underlying structures, the bone and cartilage are then reshaped and the skin re-draped over the surface. Patients who have a deviated septum may benefit by correction of this problem at the time of surgery.

Duration of the operation:
The procedure usually takes about 1-2 hours.

Post operative care:
The nose will be covered with either plaster or a light metal or plastic splint to maintain the position of the nasal bones.  You will feel some pain and discomfort at the nose area during the first days after surgery but can be alleviated by medications prescribed.

Recovery period:
You can go back to work or return to school within 2 days following surgery and will take about 1-2 weeks until it will be completely healed.

Possible risks and complications:

The implanted material is biocompatible and extremely safe.

After surgery, the physician will lubricate your eyes with ointment to reduce dryness in this area. Your vision may be blurred temporarily from the ointment. The first evening after surgery, you should rest quietly with your head elevated. It will help to apply cold compresses to your eyelids. Your blood pressure should be monitored to avoid bleeding complications that can affect vision. And, although you can be up almost immediately, you should limit your activities.

Recovery period:
In general, bruising and swelling are less than with other types of eyelid surgery, and recovery is rapid. A small amount of residual swelling, however, will persist for some months and resolve very gradually.

Significant complications from aesthetic eyelid surgery are infrequent. Lower eyelid blepharoplasty generally takes about one to three hours to complete, depending on the technique used and the extent of repair.

Post operative care:
After surgery, the physician will lubricate your eyes with ointment to reduce dryness in this area. Your vision may be blurred temporarily from the ointment. For the first 3or 4 weeks, you should avoid any activity that increases blood flow to the eyes, including bending, lifting, crying and sports.

Possible risk and complications:
Complications are rare; however there are risks associated with any medical procedure. Possible complications include: infection, bleeding (rare) but can cause loss of vision, prominence of the scars, asymmetry in healing or scarring, dryness, decrease sensation in the eyelid, difficulty closing the eyes, or any reaction to anesthesia and in rare instances blindness. You can help minimize certain risks by following the advice and instructions of your plastic surgeon, both before and after your eyelid surgery. Pre-existing conditions can also put you at risk. They include thyroid problems such as hypothyroidism, insufficient tear production or dryness of the eye, elevated blood pressure or other circulatory disorders, cardiovascular disease, and diabetes.

Overview of the surgery:
Lateral Eyebrow Lift corrects the drooping of the outer brow line. This technique is also known as a temporal brow lift, and addresses sagging around the outside of the eyebrow almost exclusively. Lateral brow lifts can be performed in conjunction with blepharoplasty or other procedures.
During a lateral brow lift, incisions are placed in the hairline at the temples towards the sides of the face. The lateral brow lift removes excess skin, lifting the outer corners of the forehead. Unlike a traditional brow lift, a lateral brow lift will not raise the middle of the brow, but it will somewhat alleviate crow's feet (the creasing at the outer corners of the eyes). Lateral brow lifts are ideally suited to prospective patients who are only concerned with the drooping of the ends of the eyebrow.

Duration of the operation:
The procedure normally takes about 20-30 minutes.

Post operative care:
When your procedure is completed, your forehead may be taped and your head may be loosely wrapped with a soft compressive dressing usually for 1-2 days to minimize swelling and bruising. A thin tube may be present to drain any excess blood or fluid that may collect under the skin. You may experience little or no postoperative pain for the first few days. Analgesic medications for pain and antibiotics to prevent infection will be prescribed.

Recovery Period:
Swelling in the eyelids can be profound but is usually short lived. Most patients can go back to work in about a week. Sutures or staples maybe remove at 5-7 days post op.

Overview of the surgery:

Incisional suturing technique is the type of cosmetic surgery procedure where the skin around the eye is reshaped to create an upper eyelid with a crease (double eyelid) from an eyelid without a crease (single eyelid). The method of suturing is the most common in Asian double eyelid surgery. The skin is incised with a scalpel at a desired height. A small strip of skin above this initial incision is excised using scissors. The incision is carried deeper into the eyelid through the orbicularis muscle and orbital septum until the orbital fat is exposed. Small strips of muscle and fat are excised. The final crease height and shape are the result of both selective tissue removal and precise internal tissue rearrangement. The incisional approach is considered the "gold standard" in Asian double eyelid surgery.

Duration of the operation:
The surgery lasts less than an hour.

Post operative care:
After your surgical procedure is already completed, cold compress and lubricating ointment may be applied to the surgical area to control the discomfort of swelling, bruising, dryness of the eyes, and in some cases your eyes may be loosely covered with gauze.

Recovery Period:
Sutures are placed and removed anywhere from 5-7 days after surgery and will usually take about 7-10 days recovery period. Activities and exercise can be resumed about 1 month after surgery.

Possible Risks and complications:
Complications are rare, however, there are risks associated with any medical procedure. Possible complications include Ptosis (asymmetrical eyes), hollow eyes wherein too much fat were removed that will result to incapability of the patient to close eyes fully and scarring. These risks can be easily corrected with surgery

Anesthesia Use:
Incisional suturing technique can be performed under local anesthesia with minor sedation or with general anesthesia.

Non-Incisional Suturing Technique


Overview of the surgery:
Non-Incisional Suturing technique is a minimally-invasive method that employs two sutures on each upper eyelid that are tied in three places and twisted around each other four or five times at the central tie. The threads can be tightened and the drooping upper lid can be elevated to a better position in more secure and therefore longer lasting results with no dissection of eyelid tissues is required.

Duration of the Operation:
The procedure can be done quickly, usually 15-30 minutes.

Post operative care:
You should rest preferably in bed on two pillows or in a recliner.  A bandage over the eyes is not needed. In the evening, you may take a bath. Avoid straining or bending over.  Watching television is acceptable. Apply clean (not sterile) cold compresses to the eyelids to prevent swelling. Pain after surgery is usually mild. You will be given pain medication.

Recovery Period:
It is normal for the eyelids to become red, swollen, and bruised for a few days and will gradually subside.  A small amount of bloody fluid draining from the wound is normal for a few days.  If you were given an antibiotic drop or ointment, gently apply a small amount to any stitches before going to bed, and then twice a day thereafter.

Overview of the procedure:
Open Upper Blepharoplasty is a popular type of cosmetic surgery procedure that removes excess skin, fat or muscle from droopy upper eyelids.
The upper eyelid incision is made in the natural crease line that occurs 1 cm above the eyelashes. The incisions might extend into the crow's feet or laughter lines at the outer corners of your eyes. Working through the incisions your surgeon will separate the skin from the subcutaneous layers and remove excess fat and trim sagging muscle and skin. The incisions will then be closed with very fine sutures.

Duration of the operation:
Open Upper Blepharoplasty usually lasts for about 30 minutes to 1 hour.

Post operative care:
After surgery, you should rest quietly with head elevated. Oral medications will help with pain and discomfort. The doctor will also prescribe an antibiotic to prevent infection. Instructions to follow for several days, includes: proper way to clean your eyes, use of cold compresses to minimize swelling, use of lubricating eye drops, head elevation, no makeup, no alcohol, no television, and no contact lenses. You should also limit activities for a few days.

Recovery Period:
Stitches are removed about a week after surgery. Bruising and swelling will continue to gradually subside after stitches are removed. Bruising usually disappears within 7-10 days duration. Sunglasses and protective sun block are required for several weeks. Normal activities can resume after about 10 days, though strenuous activities such as lifting, bending and exercise can resume after about three weeks. Most patients are back to work within 1-2 weeks.
